If you’re looking for community support for an adult with learning disabilities, autism, mental health needs or behaviours of concern, we’d be happy to talk. To discuss a potential placement or send a referral, please get in touch using the email address below:
Call or email us with your referral on 01634 869200 or referrals@clbd.org.
We’ll review the referral and any associated documentation.
If the referral appears suitable, we’ll contact the appropriate people to arrange to meet the individual and complete an initial assessment of need. From this, we’ll gain a picture of the best possible placement within CLBD and whether we have availability.
We’ll produce a recommendation of support, including a funding proposal detailing the support hours required, and send this for approval.
Once funding is approved, we’ll agree a move-in date and start planning the transition process.
